Fee Structure

For your IVF procedure, you can expect everything you've come to count on from Kaiser Permanente — quality, coordinated care, and peace of mind.

Single-cycle plan pricing

One all-inclusive fee includes:

  • Facility and physician fee
  • Lab tests
  • Cycle monitoring
  • Ultrasounds
  • Medication*
  • Egg retrieval
  • Anesthesia
  • Sperm preparation
  • Embryo culturing
  • Embryo transfer
  • Embryo cryo-storage for up to 6 months

The single-cycle plan fee does not include:

  • Additional Medication*
  • Assisted hatching
  • Intracytoplasmic Sperm Injection (ICSI)
  • Reproductive urologic procedures
  • Frozen embryo transfer

* Your infertility physician may increase or change your medications or dosage which may increase the IVF medication fee. Medication fee based on Kaiser Permanente membership status.



Payment Options

You can use your employer-provided Flexible Spending Account (FSA) or Medical Savings Plan (MSP) to help pay for IVF. Consult your employer’s health plan administrator for details about FSAs or MSPs. Payments may be made by cash, or credit card (Visa, MasterCard, American Express, CareCredit), debit card, or by cashiers check or money order made payable to The Permanente Medical Group.

Cancellation and refund policy

If you or your physician decide to discontinue the cycle at any point, all charges will be itemized and you will receive a refund for the services that you did not receive.
