What is included in the cost of an IVF procedure?

Our IVF price is comprehensive. You are charged a base fee for each IVF cycle,*
which includes the following:



The single-cycle plan fee does not include:

  • Cycle management
  • Lab tests
  • Ultrasounds
  • Anesthesia
  • Egg retrieval
  • Sperm preparation
  • Embryo transfer
  • Embryo culturing
  • Facility and physician fee
  • Embryo storage for up to six months†
  • Assisted hatching
  • Frozen embryo transfer
  • Egg donation
  • Intracytoplasmic sperm injection (ICSI)
  • Medication
  • Mind-Body Program
  • Preimplantation genetic diagnosis (PGD)
  • Testicular sperm extraction (TESE)

* Please note that your final cost may vary depending on your individual treatment requirements. A financial counselor will discuss these specific costs and payment options with you. Medication is not included.

Payment Options

You can use your employer-provided flexible spending account (FSA) or medical savings account (MSA) to help pay for IVF. Consult your employer's health plan administrator for details about FSAs or MSAs. Payments may be made by cash, credit card (Visa®, MasterCard, Discover, American Express), debit card, Chase Health Advance or cashier's check or money order made payable to The Permanente Medical Group.

Cancellation and refund policy

If you or your personal fertility physician discontinues the procedure at any point during the cycle, all charges will be itemized and any overpayment will be refunded.

Many Kaiser Permanente members have coverage for the diagnosis and treatment of infertility, leading up to, but not including in vitro fertilization services. Some employer groups and members have coverage for in vitro fertilization (IVF) procedures if IVF is listed as a covered benefit in the Evidence of Coverage for their Kaiser Permanente health benefit plan. (Please contact your employer’s Human Resources Benefits Department to review your benefit eligibility.) Otherwise, the services described are provided on a fee-for-service basis, separate from and not covered under your health plan benefits. Clinical services are provided by providers or contractors of The Permanente Medical Group, Inc. Kaiser Foundation Health Plan, Inc., and Kaiser Foundation Hospitals may receive compensation for providing facilities and/or other support in connection with these services.
